On 12^th September 2023 St Martins Practice launches Appointment Hub. We know from your feedback that it has been difficult to contact us by phone recently. In response, we are launching Appointment Hub: to make an appointment with the GP or nurse all patients will be asked to complete an online consultation form first. The online forms will then be reviewed by our hub team who will work out the necessary timescale for the appointment, and the most appropriate healthcare professional. The Hub Team consists of a GP, Nurse and Care Navigator working together as one team.

We recognise that this is a big change; we feel that it is going to be a huge improvement. We have worked hard to design and set up the new Appointment Hub. It will provide a more responsive and consistent service and will stop the “8am rush”.

The online form can be done by smartphone or a computer, by the patient or by their carer or parent. Patients without a smartphone or a computer can phone or visit us and then our Patient Support Team will complete the online form on their behalf. This is quicker & more efficient than phoning the practice.

All online forms received by 11am Monday to Friday will get a same day response. This may be by text message, by phone, or by booking an appointment. If a doctor decides that same day care is needed, then a same day face to face appointment or telephone advice will be arranged.

Patients with the greatest care need will be seen first. This system ensures that the practice manages its workload safely and efficiently.
