Cancer care support service
Our Community Cancer Support Service supports people who have a diagnosis of cancer, providing a link between hospital care and local community care (your GP practice and the District Nurse team). Whereas the hospital teams focus on diagnosis and treatment of cancer this service provides support closer to home, aiming to help patients regain control of their lives.
They will support you to:
- Reach your goals and what matters to you
- Get clear information and advice when you need it, in the way you need it
- Move forward with your life
- Review your personalised care plan at any time
You can contact the service directly using the number or email below. Alternatively, we can do so on your behalf just call or email the surgery and ask for a message to be passed to the GP you last saw or spoke to that you would like the referral made. There is no need to book an appointment in order for us to make the referral, although of course if you also have other concerns or queries then please do.
