Come and join us for fun and games at “More Than Just a Coffee Morning.”
Thursdays 10:00 – 12:00 at the Polish Centre on Chapeltown Road.

Carers Clinic
Carers Leeds provides one to one support to carers by offering emotional as well as practical support. They help carers navigate through the system, in order to access appropriate services and provision, including welfare benefits. The carer’s clinic is with Carrie Grey from Carers Leeds and is offered within St Martins Practice once a month on a Thursday.

Citizens Advice Leeds
Citizens Advice Leeds provides free confidential and impartial advice about money, benefits, housing or employment problems. There is a weekly session based at our practice. To book into it please contact Citizens Advice Leeds on 0113 223 4400, or by email or webchat through their website here.
Connect Well Wellbeing Service
We believe that everyone can live a fulfilled life however, sometimes people need help to achieve this. Feeling connected to your community has lots of health and wellbeing benefits that support resilience, independence and good physical and mental health. Aparjita is our Wellbeing Co-ordinator from Connect Well who works with you to connect you to services and activities in your community to benefit your health and wellbeing.
Our patient support team can give you information about seeing Aparjita our Wellbeing Co-ordinator.
DWP Patient Coach
Kathryn is the DWP patient coach at St Martin’s, on Monday afternoons. Her role is to support you in thinking about work / volunteering and / or training. She can provide a personalised and supportive service to help you plan your next moves. She can also talk to you about how Universal Credit works and the support you can get from your job centre patient coach, together identifying the barriers that may be stopping you from working and plan ways of overcoming these.

Healthy Minds
If you are feeling low, irritable, anxious or stressed, are experiencing poor sleep and/or concentration or have had a recent bereavement then Jenny, the Healthy Minds practitioner, may be able to help. Jenny can also help support you if you are waiting to be seen by IaPT or have been seen previously and feel that things are getting worse again.
The Healthy Minds practitioner will call you on the phone (an appointment lasts 20 minutes) and can talk through what’s troubling you and possible solutions to the problem, directing you to other support or self-help intervention if needed.

St Martins Men's Group
Having fun with games and conversation, plus peer to peer support, are the aims of these sessions.
There are free refreshments, including healthy snacks, and the group meets in the relaxed setting of our big room on the 3rd floor.
If you would like to come along, either turn up between 10:00 and 12:00 on Mondays.
